When Families Call a Hospital Notary
The most common documents we notarize in medical facilities include powers of attorney, health care powers of attorney, living wills, vehicle title transfers, and bank or retirement account forms. These documents often let a trusted family member handle bills, medical decisions, or property while the signer recovers.
What the Signer Needs
The person signing must be able to understand the document and sign it willingly. A notary cannot notarize a signature if the signer appears confused, heavily sedated, or unable to communicate their wishes. If medication affects alertness, it helps to schedule the visit for a time of day when the signer is most clear-headed.
The signer also needs a current government-issued photo ID, such as a driver's license, state ID, or passport. If the ID is missing or expired, Arizona law allows identity to be confirmed through credible witnesses. Call ahead and we will explain what is needed.
How to Prepare for the Visit
- Have the documents printed and ready, but leave them unsigned. - Check whether the document requires a witness. Hospital staff are often not permitted to serve as witnesses, so plan to have an eligible adult present. - Confirm the room number, visiting hours, and any check-in rules with the facility. - Make sure every person who needs to sign will be there at the scheduled time.
